General Description

Department of Medicine - Infectious Diseases / Moore Clinic
Work Schedule/Hours: 1 day a week/3 hours a day


The successful candidate for this position will facilitate educational/support groups in the Bartlett Specialty Practice for clients diagnosed with HIV/AIDS that have multiple medical and psychosocial needs. Prepare group lesson plans and topics for discussion. Collaborate with clinic’s HIV case managers in the group recruitment process. Assess client psychosocial needs and develop a plan of care for clients participating in group. Refer clients as needed to additional resources in the community that address their psychosocial needs. Document progress notes in EPIC patient medical record system. Order food for group members. Complete all paperwork in accord with current policies, act as liaison with Bartlett Specialty Practice interdisciplinary team as needed, participate in relevant training, clinical supervision, conferences and meetings, maintain operative knowledge of existing resources for individuals with HIV/AIDS, and complete other duties as assigned.

Duties and Responsibilities:

Note: Will report to the Clinic SW/CM Supervisor and the Director of Social Work, JHU AIDS Service.

Qualifications

Bachelor’s Degree in social work, psychology or other appropriate field. Three years of case management experience in a healthcare or public welfare setting required. Additional related graduate education can substitute for some required experience to the extent permitted by the JHU Equivalency Formula.

Special Knowledge, Skills, and Abilities:

  • Demonstrate general knowledge of HIV disease, treatment, and resources.
  • Knowledge specific to public entitlement programs related to health care.
  • Able to work with culturally diverse patient, staff and community populations.
  • Strong organizational skills with attention to detail and follow through.

    JHU Equivalency Formula: 18 graduate degree credits may substitute for one year of experience. .

  • Preferred Qualifications

    Master’s Degree in the field of Social Work, Psychology, or related field.
    Knowledge of HIV, substance abuse, and psychiatric illness.

    The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
