CO Clinical Nurse

We are seeking a CO Clinical Nurse who will coordinate a variety of clinical activities for patients of the Johns Hopkins Comprehensive Care Practice. This position is located in the Division of Addiction Medicine at the Johns Hopkins Bayview Medical Center.


Specific Duties & Responsibilities

  • Coordinates care for complex primary care patients to achieve clinical and organizational outcomes associated with value based programs such as MD PCP (Maryland Primary Care Program).
  • Provides primary nursing care to clinic patients. Provides phone triage during clinic hours and help physicians with between-visit management of symptoms and medications. Prioritize and route messages as appropriate to the physicians.
  • Performs transitional care management calls for practice patients discharged from hospitals and emergency departments in the area and determines the need for short-term outpatient follow-up.
  • Assists primary providers with maintaining electronic medical records: scans outside results into Epic EHR system, updates immunization history, updates health maintenance records, acts on overdue health maintenance items.
  • Helps obtain prior authorizations for tests and medications.
  • Successfully manages fast-paced clinical environment with ability to negotiate frequent interruptions in planned work.


Technical Qualifications or Specialized Certifications:

  • Current Maryland State RN Licensure.
  • Completion of the JHU credentialing process to include verification of CPR certification is required prior to start date.
  • Must maintain current licensure and certification during duration of employment.


Physical Requirements 

  • Ability to stand, walk or sit for an extended period of time.
  • Finger Dexterity required to manipulate objects with fingers rather than with whole had(s) or arm(s), for example, using a keyboard.
  • Ability to see within normal parameters.
  • Ability to hear within normal range.
  • Ability to lift 15 pounds.
  • Possible exposure to communicable diseases.


Minimum Qualifications
  • Individual must be a registered nurse, licensed in the State of Maryland or state where practicing.


Preferred Qualifications
  • Bachelor's Degree in nursing or related discipline preferred.

30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers.
