CO Clinical Nurse

We are seeking a CO Clinical Nurse to support the General Internal Medicine (GIM) Faculty practice at the Johns Hopkins Outpatient Center (JHOC).


*This position is part of the JHU School of Medicine Clinical Operations.


Specific Duties & Responsibilities

  • Responsible for other clinical duties as assigned.
  • Helps obtain prior authorizations for tests and medications when unable to obtained by MOC.
  • Helps complete forms (e.g., disability, FMLA, mobility) requiring basic health history.
  • Performs transitional care management calls for practice patients discharged from hospitals and emergency rooms in the area and determines the need for short-term outpatient follow-up.
  • Provides patient education for self-care chronic disease management (in person, by phone, or via electronic record).
  • Assists Medical Office Coordinator (MOC) in scheduling patients for consult appointments.
  • Conveys critical action values/patient request of lab results when requested by provider (communication from MD to RN)
  • Assists primary providers with maintaining electronic medical records; scans outside results into system, updates immunization history, updates health maintenance records, acts on overdue health maintenance items.
  • Identifies care continuity needs of patients; generates and pends orders to primary providers for signature.
  • Triages calls from outside institutions, the JHH Emergency Department, and other health care providers; determines and implements follow-up plans.
  • Assesses lab results and implements intervention in consultation with primary providers.
  • Triages phone calls and electronic advice requests from patients and determines need for medical assessment.
  • Works collaboratively with a multidisciplinary health care team to optimize team performance and patient outcomes.
  • Actively participates in Comprehensive Unit Based Safety Program (CUSP) initiatives.
  • Provides direction to/management of other team members to promote quality, safety, and excellence in care.
  • Provides evidence-based care based on nursing process, policies, procedures, and protocols of the Johns Hopkins Hospital.
  • Coordinates care for a group of primary complex patients to achieve clinical and organizational outcomes.


Scope of Responsibility

  • Knows the formal and informal departmental goals, standards, policies and procedures which may include some familiarity of other departments within the school/division. Is sensitive to the interrelationship of both people and functions within the department.


Decision Making

  • On a regular and continuous basis, exercises administrative judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the functional area.


Communication

  • Exchanges non-routine information using tact and persuasion as appropriate requiring good oral and written communication skills.


Knowledge

  • Knowledge of nursing process; social, behavioral, biological, and physical sciences; safety and scientific inquiry process and application to the skill performance; leadership and management as normally acquired through completion of a bachelors program in nursing.
  • Knowledge of current health care environment.
  • Knowledge of general physiologic monitoring and patient care equipment such as defibrillator and glucometer monitor.
  • Successfully completes all training courses required for access to Epic and other clinical software systems.
  • Requires effective use of resources and cost awareness.



Minimum Qualifications
  • Individual must be a registered Nurse, licensed in the State of Maryland or state where practicing.
  • Bachelor's Degree in Nursing or related discipline preferred.

Additional Minimum Qualifications:

  • Completion of the JHU credentialing process to include verification of CPR certification is required prior to start date.
  • Must maintain current licensure and certification during duration of employment.

The referenced base salary range represents the low and high end of Johns Hopkins University’s salary range for this position. Not all candidates will be eligible for the upper end of the salary range. Education and Experience Equivalency
Please refer to the job description above to see which forms of equivalency are permitted for this position. If permitted, equivalencies will follow these guidelines: JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination.
