CO Nutritionist Supervisor

 

The mission of the Johns Hopkins Brancati Center for the Advancement of Community Care is to implement evidence-based programs in partnership with community organizations to improve the health of the community. The Brancati Center works cooperatively with community-based organizations such as churches, recreation centers, and clinics to offer in-person, distance learning, and virtual National Diabetes Prevention Programs (DPP) to their members. The Brancati Center is an approved Medicare and Medicaid DPP provider.  The Brancati Center maintains high standards under the CDC’s National Diabetes Recognition Program by holding full recognition status. The Brancati Center team includes clinical and professional staff, lifestyle coaches, community health workers, and program coordinators. 

 

General Summary:

Reporting to the Brancati Center’s Program Administrator and the Brancati Center’s Director of Community Partnership, the Nutritionist Supervisor will participate in the development, implementation, and evaluation of Johns Hopkins Brancati Center community-based in-person and distance learning Diabetes Prevention Programs. Specific areas of responsibility include DPP program implementation, operations oversight, data monitoring and reporting, administrative supervision, personnel supervision, billing and compliance, and related tasks. The Nutritionist Supervisor will also assist with program development and implementation of quality assurance measures to ensure the DPP achieves quality outcomes. The Nutritionist Supervisor will work with the Brancati Center Program Administrator to ensure all DPP operations meet CDC and CMS requirements.  The Nutritionist Supervisor will directly interact with program participants, community partners, the DPP team, including the DPP Coordinator, DPP Lifestyle Coaches, Community Health Workers, and other center members and faculty involved in the implementation of DPP-related initiatives. The Nutritionist Supervisor will also work with public health officials, physicians, payer communities and other referral networks to enhance awareness of and referrals to the DPP. 

 

Program Implementation:

Under the direction of the Program Administrator and the Director of Community Partnership, the Nutritionist Supervisor will oversee all DPP groups, operations, and initiatives related to the Brancati Center’s in-person and distance learning DPP. The Nutritionist Supervisor will evaluate effectiveness of the DPPs while proposing and revising processes as necessary to achieve successful program outcomes. The Nutritionist Supervisor will also supervise the Brancati Center’s DPP staff. 

 

Specific Duties:

  • Implement procedures for the recruitment, screening, and enrollment of participants into the DPP groups. Evaluate the effectiveness of procedures while proposing and revising processes as necessary.
  • Assist with the development of communication and marketing tools for program recruitment. Work with Brancati Center DPP staff and Program Administrator as needed.
  • Participate in participant recruitment along with the DPP team as needed.
  • Establish program timelines for all stages of DPP implementation:  recruitment, screening, enrollment of participants, group delivery, and data reporting.
  • Develop and refine program goals, timelines, and procedures for accomplishing DPP projects. Consult with Senior Interventionist, Director of Community Partnership, and Program Administrator as needed to accomplish all projects successfully.    
  • Lead DPP team meetings to ensure all details on DPP-related projects are communicated and reviewed on a timely basis.
  • Lead DPP intervention case management meetings for in-person and distance learning DPPs to ensure that staff have the resources needed to help participants meet program goals.
  • Supervise the DPP Coordinator and DPP Lifestyle Coaches from launch to completion of DPP groups.
  • Ensure adequate class coverage with staffing/group schedules.
  • With the Data Manager, refine internal tracking systems for evaluating and reporting program outcomes.
  • Oversee staff to ensure accuracy of data collection (e.g., measurement of weight) and integrity of data for the DPP. Oversee resolution of DPP data discrepancies.
  • Review program data to ensure it meets quality performance standards.
  • Work closely with the Data manager to ensure maintenance of CDC database and compliance with CDC Diabetes Prevention Recognition Program data submissions.
  • Provide support and guidance to the coaches and participate in routine coach collaboratives to monitor the lifestyle program participants and program outcomes.
  • Monitor the retention and commitment of lifestyle program participants. Serve as point person for participant concerns.
  • Work closely with the Program Administrator to develop and implement billing processes for the Diabetes Prevention Program. 
  • Demonstrate a thorough understanding of the National Diabetes Prevention Program, Medicare Diabetes Prevention Program, and HealthChoice Diabetes Prevention Program rules and regulations. Implement processes in accordance with these regulations.
  • Learn and implement new reimbursement procedures as they arise (e.g., new payer covering DPP).
  • Serve as a liaison for the JHU DPP lifestyle program within public health, physician, health care professional, and payer communities.
  • Work with potential partners at new community DPP sites and assist them to put into practice the systems, tools and policies that have been developed to ensure operational efficiency and quality assurance compliance.
  • Become trained as a certified CDC Master Trainer. Conduct Lifestyle Coach trainings in collaboration with the Senior Interventionist.
  • Ensure compliance with JHU operational policies, applicable laws and regulations and those governing privacy and data security (HIPPA regulations).
  • Oversee DPP-related communications between DPP staff and referring clinicians.
  • Collaborate with senior staff and Director of Community Partnership in the drafting, submission and maintenance of IRB documents.

Supervisory Duties:

  • Serve as the supervisor for DPP Coordinators, lifestyle coaches, assistants and other administrative personnel. This includes developing job descriptions, hiring, evaluation, disciplining and termination.
  • Determine and propose staffing projections while adhering to budget limitations.
  • Provide leadership and foster team cooperation with a commitment to program quality. 
  • Support professional growth, skills development, and career development.  Monitor the quality of support that lifestyle coaches provide to program participants.

 

Lifestyle Coach (back-up capacity):

  • Guide behavior change efforts with a knowledge of the principles of behavior change, including motivational interviewing techniques.
  • Communicate empathy for participants.
  • Build strong relationships with individuals and build community within a group.
  • Administer all aspects of delivering the lifestyle intervention.
  • Understand and oversee all aspects of participant safety-related issues with respect to program delivery.

 

 

Minimum Qualifications (Required):

Education: Registered dietitian and Master’s degree in nutrition, public health, or health care related field.

 

Experience: Three years’ experience in conducting behaviorally oriented diet/exercise groups and counseling sessions, clinical experience with diabetes, hypertension, and/or obesity; familiarity with motivational interviewing, and sensitivity to the needs of minority populations.    

Experience with delivering interventions in community-based setting in Baltimore City metropolitan area preferred.

 

 

Special Skills and knowledge: 

Excellent computer skills:  Microsoft Office Suite

Familiarity with EPIC and REDCap preferred. 

Excellent organizational skills with specific attention to detail and data collection. 

Able to work independently as well as collaboratively as a team member.

Outstanding verbal communication and interpersonal skills with participants and other team members.

A friendly demeanor with strong customer service skills.

Flexibility to work with people from all walks of life.

Has access to reliable transportation.

Able to work some evenings and Saturdays with variable work conditions.

 

Physical Requirements:

Sitting in a normal seated position for extended periods of time. Reaching by extending hand(s) or arm(s) in any direction. Finger dexterity to manipulate objects with fingers rather than with whole hand or arm, for example, using a keyboard. Ability to move about workspace and to lead group classes.  Ability to carry light equipment (easel, flip charts, etc.).
