Clinical Case Manager

Student Health and Well-Being (SHWB) Mental Health Services at Johns Hopkins University seeks an energetic, organized, and motivated Clinical Case Manager to work in a dynamic and fast-paced environment focused on supporting the well-being of a diverse community of learners. Reporting to the Assistant Director of Case Management, the Clinical Case Manager is a member of the team focused on providing support for referrals, assistance with medical leaves, and care coordination within the SHWB suite of services.


The Clinical Case Manager will assist students and learners with transitioning to community providers for longer-term or specialized care; work collaboratively with primary care, psychiatry, and clinical staff to ensure continuity of treatment; establish and maintain a robust community referral network; provide education around student insurance and benefits; and support students transitioning to and from higher levels of care. The successful candidate demonstrates a commitment to diversity, equity, inclusion, and belonging in clinical work and all professional activities.


Specific Duties & Responsibilities


Case Management Duties (85%)

  • This position’s case management activities will be based in both Maryland and Washington, D.C. clinics, Licensure in all jurisdictions is required within 12 months of hire.
  • Assists students and learners with connections to community providers, including therapists, psychiatrists/prescribers, and ancillary resources.
  • Investigates insurance information relevant to the referral process on behalf of clients and provide psychoeducation on insurance benefits.
  • Follows up with students and learners to ensure continuity of care, reviews feedback from students and learners about their experiences with community providers.
  • Creates, develops and maintains relationships with outside referral sources, particularly focusing on developing a network of providers within proximity of JHU campuses.
  • Collaborates with clinical staff to design individual care plans when a higher level or specialized care is clinically indicated or requested by clients.
  • Coordinates post-hospitalization communication, treatment planning and referral with the care coordination team, students/learners and their support systems.
  • Coordinates follow-up for Patient Health Questionnaire (PHQ-9) referrals from SHWB Primary Care.
  • Coordinates medical leave, withdrawal and readmission processes for students and learners; collaborates with relevant student affairs and division partners throughout leave processes.
  • Assists in the development of policies and procedures related to leave processes.
  • Represents Mental Health Services on the University Insurance Committee.
  • Processes insurance referral paperwork, deductible waivers and related documentation for the student health insurance plan.
  • Collaborates with providers with SHWB as an interdisciplinary treatment team.
  • Other duties as assigned.


Collaborative & Consultative Activities (10%)

  • Provides consultation to student affairs leaders, faculty, and staff related to medical leave processes and care coordination for community referrals.
  • Participates in MHS team meetings, recruitment and hiring processes.
  • Provides training to new staff members and trainees related to policies and procedures for community referrals and leave assistance.
  • Provides outreach programming to the University community regarding the case managers’ roles, including how to direct students to the Mental Health Services.
  • Participates in outreach to campus department and incoming students.


Outreach Duties (5%)

  • Participates in outreach and educational activities, including resource fairs, community meetings, and training for JHU faculty, staff, and students.

Schedule

  • 37.5 hours per week (not including lunch breaks). Some after-hours and weekend work is expected based on the programmatic needs of the University during the academic year (e.g., orientation, special events, etc.) This position is eligible for some remote work, schedule to be determined by clinic needs and supervisor approval.
  • This position is designated as a Required Attendance Employee, designated as vital to the operation of a work group, so may be required to work during a university closure in response to operational needs.


Minimum Qualifications
  • Master’s Degree in Social Work, Clinical Psychology or Counseling required.
  • Licensed as a mental health provider in required jurisdiction(s) noted in the job description (LMFT, LCPC, LCSW-C) or license-eligible (must obtain licensure in required jurisdiction(s) within 12 months of hire).
  • Three year's experience in the field of clinical psychology, counseling, or social work.
  • Prior experience providing referrals for mental health services.
  • Experience interacting with mental health providers and insurance companies.
  • Experience working in an interdisciplinary work setting.


Preferred Qualifications
  • Experience in a university setting strongly preferred.
  • Experience working with international students strongly preferred.
  • Proficiency in Mandarin strongly preferred.

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing.
