Clinical Nurse (Liver MDC)

General Summary/Purpose:

The clinical nurse is responsible: Coordinating care for assigned patients to promote the achievement of clinical outcomes. Provide direct patient care based on the nursing process. Collaborate with a multidisciplinary health care team to optimize team performance. Promote improvements in treatment area processes and cost effective use of resources. Accountable for own decisions regarding patient care. Identifies impending changes in patient condition and individual performance issues. Analyzes patient care data to deliver and evaluate care

 

Duties and Responsibilities:

  • Coordinating care for assigned patients on a shift to promote the achievement of clinical outcomes.
  • Providing direct patient care based on the nursing process.
  • Working collaboratively with a multi-disciplinary health care team to optimize team performance.
  • Promote improvements in unit processes and cost effective use of resources.
  • Accountable for own decisions regarding patient care
  • Identifies impending changes in patient condition and individual performance issues
  • Analyzes patient care data to deliver and evaluate care

 

Supervision of Others: 

  • May supervise 1 MOC

 

Equipment, Machine, or Tool Requirements: 

  • General physiologic monitoring and patient care equipment such as defibrillator and glucometer monitor.

Minimum Qualifications:

Education:

  • Required: Associate Degree or diploma in nursing from an accredited school of nursing.
  • Preferred: Bachelor’s Degree in Nursing (BSN)

 

Licensure, Certification, Registration:

  • Registered nurse licensure in the State of Maryland

 

Special Knowledge, Skills, or Abilities / Competencies:

  • Knowledge of nursing process, social, behavioral, biological and physical science
  • Knowledge of current health care environment
  • Ability to analyze data to synthesize and summarize information and make judgments regarding patient care
  • Highly effective verbal communication and interpersonal skills to establish working relationships that foster optimal quality patient care
  • Ability to document legibly using a writing instrument
  • Ability to use a computer keyboard to input and obtain data
  • CPR and AED certified
  • Ability to properly conduct general physiologic monitoring and patient care equipment such as an EKG.

 

Work Conditions:

  • Possible exposure to communicable diseases, hazardous materials, radiation and pharmacological agents.
  • Work requires standing and/or walking for extended periods of time.
  • Work requires lifting and/or positioning patients and transporting wheelchairs, beds and related equipment.
  • Work requires manual dexterity and hand-eye coordination to perform patient care procedures.
  • Work may require contact with aggressive and/or combative patients.
  • Employees in this position will be required to adhere to work availability and scheduling requirements.  This may include, but is not limited to availability status, mandatory overtime, on call and shift work.

Liver Cancer Multi-Disciplinary Clinical Coordinator

 

Position Summary: The Multi-Disciplinary Clinical Coordinator for care, outreach and education of the liver multidisciplinary cancer program will be responsible for enhancing patient and provider satisfaction, improving access to services, and assuring that patient data is captured and shared among the team members.  In addition, this person will be charged with providing materials and programming to increase awareness and education about this particular tumor type.  This individual will serve as an integral member of the multidisciplinary team, working closely with the program champion, and a wide ranging group of specialists and providers.  Further, the coordinator will be responsible for maintaining or developing a way to collect, analyze and present data related to the size and scope of the program, and will be knowledgeable about the particular tumor, cancer in general, the program and Johns Hopkins research related to this tumor. This position will be expected to be a part of the survivorship efforts in liver cancer.

 

  • Triage new patients to members of the liver cancer team by review of medical records, phone history and review of scans
  • Manage return and follow up patients with team members
  • Pend blood tests and radiology to the triage attending, when appropriate
  • Coordination of Liver Multi-Disciplinary Clinic
  • Coordination of care between disciplines
  • Serve as link to research nurses and coordinators for clinical trials
  • Coordinate with Marketing and Development office for fundraising efforts

 

In addition, the nurse will have a clinical role in the liver cancer survivorship program, supporting the faculty in providing care, education, and support to patients. This role will include patient assessment, patient follow up, patient education, and patient support with a focus on long-term care and surveillance. Many of these patients will undergo complex treatment regimens, on and off clinical trials, and will require care and support during and after their treatment, due to side effects of treatment and advancing disease. The nurse will become imbedded in the team, and will work collaboratively with physicians, therapists, nurses, research nurses, data coordinators, social workers, dietitians, and administrative staff to support patients.

During the Influenza ("the flu") season, as a condition of employment, The Johns Hopkins Institutions require all employees who provide ongoing services to patients or work in patient care or clinical care areas to have an annual influenza vaccination or possess an approved medical or religious exception. Failure to meet this requirement may result in termination of employment.

 

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
