Clinical Nurse Case Manager

Under the supervision of the Clinical Nurse Manager, we are seeking a Clinical Nurse Case Manager who will perform duties related to medical case management and non-medical case management nursing duties as a member of the health care and support service team providing services to people living with HIV and viral hepatitis. The candidate will counsel and manage patients on HIV pre-exposure prophylaxis (PrEP) and non-occupational HIV post-exposure prophylaxis (nPEP) under standing orders in an RN-led care model. The candidate will test, treat, and provide prevention interventions to patients for sexually transmitted infections per clinic protocols, and triage and consult with clinic providers (NP, PA, physicians) at the Baltimore City Health Department (BCHD) sexual health and wellness clinics.


Specific Duties & Responsibilities


HIV program duties

  • Case management of a panel of HIV continuity patients including monitoring and educating clients about medication side effects, disease process, and lab values.
  • Conduct psychosocial assessments, identify needs for interventions, and facilitate the ability of patients in the HIV care program to access recommended interventions, and develop treatment plans.
  • Provide HIV treatment adherence counseling.
  • Adherence assessment/Plan.
  • HIV/HAART education.
  • Pill box preparation.
  • Regular monitoring of adherence and tolerance to medications.
  • Pharmacy monitoring of prescription completion by patient.
  • Administer and coordinate long-acting injection delivery for HIV therapy. Coordinate appropriate drug storage and handling.
  • PrEP and nPEP program duties.
  • Evaluate BCHD Sexual Health Clinic patients for HIV PrEP or nPEP initiation, order baseline labs, provide education and counseling, manage prescriptions under clinician NPI number and following standing orders, work with support staff to eliminate financial and insurance barriers to PrEP, act as the main point of contact for patients.
  • Manage a panel of patients on PrEP and doxycycline PEP, lead the site-specific PrEP team through a regular process of reviewing the patient panel and conducting appropriate follow-up to support persistence on PrEP/doxycycline PEP.
  • Administer and coordinate long-acting injection delivery for PrEP therapy. Coordinate appropriate drug storage and handling.
  • Hepatitis C program duties.
  • Case management of a panel of patients being treated for Hepatitis C including monitoring and educating clients about medication side effects, disease process, and lab values.


General Duties

  • Conduct “express” STI testing visits for patients appropriate for laboratory testing only.
  • Obtain sexual and behavioral risk history.
  • Provide patient education on HIV, sexually transmitted infections, hepatitis test results, disease transmission, and risk reduction.
  • Collect specimens and coordinate with the lab when necessary.
  • Assist with performing point-of-care testing (rapid HIV, rapid Hepatitis C, etc.).
  • Review test results, communicate results to patients, and triage to providers as appropriate.
  • Administer medications and vaccines as appropriate per clinic protocols.
  • Assist with prescription refills.
  • Consult with the ordering clinician or medical director for patients with complex medical conditions or any medical decision-making that is not covered in standing orders.
  • Triage patients presenting with acute issues, by phone or in person.
  • Assist with tracking and restocking clinic medication inventory.
  • Complete all charting in medical records per protocol.
  • Additional administrative tasks as necessary.
  • Assist with clinic initiatives including QI activities.
  • Interpret for Spanish-speaking patients when medically necessary.


Minimum Qualifications
  • The individual must be a registered nurse, licensed in the State of Maryland or the state where practicing. Bachelor's degree in nursing or related discipline preferred.
  • Current CPR certification required.
  • Must maintain current licensure and certification during the duration of employment.
  • Acceptable completion of the JHH credentialing process is required prior to the start date.

  • Knowledge of STI/HIV/TB/HCV disease, treatment, prevention, and resources.
  • Ability to work with culturally diverse patients, staff, and community populations.
  • Ability to work in a multidisciplinary, multi-organizational team setting.
  • Strong organizational skills with attention to detail and follow-through.
  • Self-starter with the ability to work with minimal supervision.
  • Strong leadership potential.
  • Phlebotomy and injection skills.
  • Proficiency in medical and conversational Spanish desired.

Education and Experience Equivalency
Please refer to the job description above to see which forms of equivalency are permitted for this position. If permitted, equivalencies will follow these guidelines: JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.
