Clinical Nurse

We are seeking a Clinical Nurse to assist in coordinating the primary care of patients under direct supervision of providers. The work includes clinical triage during practice hours and transition of care for recent hospital discharges. The Clinical Nurse will identify and work with practice case managers and community health workers to actively ensure patients with treatment adherence issues related to missed appointments are contacted and scheduled for follow-up appointments.


Specific Duties & Responsibilities

  • Independently monitors CRISP notifications in Epic for hospital and discharges and communicates with medical providers regarding the need for a follow-up appointment.
  • Contacts each patient and documents all decisions in Epic regarding the need for a post hospital follow-up appointment.
  • Works with physicians and staff to improve performance in Pay-for-Performance programs, specifically MD PCP program. Attends all MD PCP meetings.
  • Provides clinical triage during clinic hours to calls and sick drop-in visits from patients requesting urgent appointments and uses nursing judgment to assess acuity.
  • Schedules urgent care (acute) appointments in clinic or refers patients to the Emergency Department, as indicated by acuity.
  • May assist with patient vitals, etc. when short staffed.
  • Maintains current knowledge of all JHU and Comprehensive Care Practice policies and procedures related to operation and assumes responsibility for appropriate application and compliance. Implements Infection Control procedures for the clinic.
  • Documents patient care interactions (nursing visits, patient education, telephone contacts, medication refills) in electronic medical record.
  • Participates in Clinical Quality Indicator (CQI) meetings and works with providers and MA staff to improve outcomes.
  • Assures that emergency and diagnostic equipment within the clinic is functional and serviced as warranted.
  • Successfully manages fast-paced clinical environment with frequent interruptions in planned work. Regularly interfaces with patients and a multidisciplinary care team.
  • Effectively utilizes electronic medical record and electronic medication prescription programs. Adapts well to advancing, evolving work environment.
  • Possible exposure to communicable diseases and hazardous materials in work setting has little likelihood of harm if established health precautions are followed.
  • Perform other duties as directed.


Minimum Qualifications
  • Registered Nurse, licensed in the State of Maryland



Preferred Qualifications
  • Bachelor's Degree in Nursing or related discipline.
  • Strong interest in and/or knowledge of Primary Care, HIV/AIDS and Substance Use Disorder care.

Not all candidates will be eligible for the upper end of the salary range. Education and Experience Equivalency
Please refer to the job description above to see which forms of equivalency are permitted for this position. If permitted, equivalencies will follow these guidelines: JHU Equivalency Formula: 30 undergraduate degree credits (semester hours) or 18 graduate degree credits may substitute for one year of experience. Additional related experience may substitute for required education on the same basis. For jobs where equivalency is permitted, up to two years of non-related college course work may be applied towards the total minimum education/experience required for the respective job.
