Clinical Nurse

General Summary:

The primary purpose of the registered nurse position, the Early Intervention Services Nurse, is the provision of HIV and pre-exposure prophylaxis (PrEP) nursing services for patients who are seen in the John G. Bartlett Specialty Practice. They will provide nurse intake, triage, and linkage to care and reengagement in care services for HIV positive patients new to the John G. Bartlett Specialty practice. They will be the main point of contact for referral of patients identified as HIV positive and not in care through other clinical programs including the Emergency Department and Inpatient services at the Johns Hopkins Hospital East Baltimore Campus. They will work with medical providers to provide comprehensive evaluation and treatment and fa8:30AMcilitate referrals to other programs for needed medical services. They will work closely with patient navigators, providers, and case managers for linkage to care and care coordination services. Major responsibilities include care coordination, patient education and instruction, and clinical assessments.

 

Specific duties and responsibilities:

PrEP activities: PrEP nurse intake visit, eligibility assessment, education and counseling, case management screening, phone and walk-in triage, adherence counseling. Implementation of self-swab for GC/CT testing in the clinic to facilitate STI screening. Provide walk-in HIV testing, counseling, and linkage-to-care services.

HIV rapid start activities: Point of contact for patients newly diagnosed/previously diagnosed reengaging care; screen for eligibility for rapid start/re-start of ART, rapid start visit care coordination, HIV nurse intake/assessment with warm hand-off to rapid start HIV provider, case manager, and navigator; HIV and ART education and adherence counseling; tracking of patients who receive rapid start to help ensure transition to HIV continuity care. Rapid start patients will transition to continuity nursing services with nurses who are part of the patient's HIV primary care provider's team let and patients will be tracked to ensure completion of at least three continuity HIV care medical appointments. This nurse will be the point of contact for rapid start collaborative activities with the Johns Hopkins Emergency Department, inpatient unit, and JHH and external clinics where HIV testing occurs.

 

Reengagement in care activities:  A monthly report from the electronic medical record (EPIC) will be run to identify HIV-positive patients who have been lost to follow up. S/he will work with Bartlett case managers, navigators and the BCHD linkage-to-care team to contact patients for re-engagement in care. S/he will perform a nursing intake evaluation including triage for urgent medical appointments, assessment of eligibility for rapid ART restart, and a review of psychosocial barriers to care with referrals to psychosocial services as appropriate.  Patients reengaging in care will transition to continuity nursing services with nurses who are part of the patient's HIV primary care provider's team let and patients will be tracked to ensure completion of at least three continuity HIV care medical appointments.  S/he will be the point of contact for re-engagement in care collaborative activities with the Johns Hopkins Emergency Department, JHH and external clinics, and other organizations where people living with HIV but not in care receive services.

 

General nursing duties:

  • Provide primary nursing care for patients.
  • Facilitate communication among the clinical care team of provider, case manager and patient. 
  • Answer calls from patients requesting emergency appointments.  Independently determine need for medical assessment and advises clerical staff to schedule appropriately.
  • Provide/direct outside agencies to obtain authorization for procedures for HIV-infected patients receiving primary.
  • Independently assess laboratory reports for abnormalities and implement an intervention plan in to resolve the problem.
  • Provide clinical triage during clinic.  Clinically assess need for diagnostic testing prior to being evaluated by the medical clinician. 
  • Assist in assessing and triaging acutely ill patients.
  • Arrange for patient hospitalizations by coordinating the interaction between the attending physician, medical shift coordinator, Infectious Diseases Fellow and inpatient unit.
  • Conduct educational sessions, individual or group, addressing HIV treatment and management issues as outlined in clinical protocols.

 

Administrative duties

  • Maintain current knowledge of all JHH and JHU policies and procedures related to unit operation.  Assume responsibility for appropriate application and compliance.
  • Attend treatment team and staff meetings.
  • Facilitate HIV education through presentations and discussions regarding specific patients.

 
Education:    

  • Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor’s Degree in nursing preferred. Current CPR certification required. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date.   

 

Experience

  • One year of nursing experience in a healthcare or public welfare setting with knowledge of HIV prevention, HIV treatment, substance abuse, and psychiatric illness preferred.
     

Special Knowledge, Skills, and Abilities: 

  • In-depth knowledge of the clinical manifestations and management of HIV infection
  • Adept at the clinical assessment of patients living with and at risk for HIV infection
  • Ability to take independent action to care for patients who are acutely ill awaiting provider evaluation
  • Strong interpersonal skills required to interact with health care providers and staff from many disciplines
  • Knowledge of lesbian/gay/gender identity issues, and resources.
  • Able to work with culturally diverse patient, staff and community populations.
  • Strong organizational skills with attention to detail and follow through.

During the Influenza ("the flu") season, as a condition of employment, The Johns Hopkins Institutions require all employees who provide ongoing services to patients or work in patient care or clinical care areas to have an annual influenza vaccination or possess an approved medical or religious exception. Failure to meet this requirement may result in termination of employment.

 

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
