Community Outreach Specialist

The General Peds Division of the Department of Pediatrics is seeking a full time Community Outreach Specialist (COS). The successful candidate will be primarily responsible for assisting medically complex pediatric patients and families, including those who receive social security income (SSI), have asthma, or are a part of maternal mental health support team, with navigating state healthcare systems, applications, and ensuring regular follow up with required appointments. Additional duties for this position will include, but not be limited to health program application assistance, screening and enrollment for eligible community programs; patient education; patient follow-up and reinforcement of treatment plan tasks; and coordination of appointments and transportation for pediatric patients at the Harriet Lane Clinic. The Community Outreach Specialist will report to the Program Coordinator of the Hopkins Community Connection Program at the Harriet Lane Clinic and will work with an interdisciplinary team of physicians, case managers, social workers, and other community health workers to improve health outcomes.


Specific Duties & Responsibilities


Outreach & Care Management

  • Assist clients in meeting selected care plan components as assigned.
  • Provide outreach and appointment follow up support to patients.


Patient Advocate

  • Educate patients of available resources.
  • Assist clients in obtaining support when necessary.
  • Act as a patient advocate and support the development of problem-solving skills; provide instructions for navigating application processes in an accessible manner.


Appointments

  • Support and implement clinic strategies to increase appointment show rates for SSI, Asthma, and Maternal Mental Health team patients through proactive outreach to patients, regular follow up, and addressing barriers to access when appropriate.


Administrative Duties

  • Remain familiar with Medical Assistance eligibility criteria, documents necessary for application submission, and necessary recertification procedures to ensure minimal lapses in health coverage for clinic patients.

Additional Skills, Knowledge, & Abilities

  • Must be able to speak, read and write in English. Working knowledge of social and health issues.
  • Familiarity with medical terminology.
  • Experience working with Electronic Medical Records.
  • Ability to work independently and efficiently.
  • If using a personal vehicle for work related travel, a valid Maryland driver’s license is required.


Minimum Qualifications
  • High School Diploma or graduation equivalent.
  • Two years related experience.
  • Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.


Preferred Qualifications
  • Familiarity with navigating health insurance and benefit programs.
  • Knowledge of Baltimore community-based organizations and resources.
  • Exceptional initiative and attention to detail.
  • Superb interpersonal and communication skills.
  • Strong skills in Microsoft Office Suite, especially Teams, Excel and PowerPoint.

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing.
