Sr. Clinical Nurse (Allegany, Frederick, and Washington Counties)
The Center for Substance Use and Infectious Disease Care Integration is seeking a full time Sr. Clinical Nurse to work with our patients in Allegany, Frederick, and Washington Counties.
The Sr. Clinical Nurse is an integral part of a multidisciplinary team working with clinicians, Community Outreach Coordinators, Peer Coaches, and rural Maryland Health Departments in Western Maryland harm reduction teams to engage and treat persons who use drugs and related infectious diseases through tight care coordination, nursing assessments, and nurse case management services.
The Sr. Clinical Nurse will be responsible for coordinating patient care through the care continuum of HIV/HCV/SUD screening, diagnosis, education, linkage to telemedicine care, on-treatment support, and post-treatment follow up. This role includes the provision of onsite and remote medical case management services as well as coordination of operations for the delivery of treatments in rural Western Maryland Health Departments. This position requires someone who is self-motivated, highly organized, works well independently as well as part of a team, and thrives in a public health environment to connect with people from a variety of backgrounds and life experiences.
The Clinical Nurse will be responsible for supporting implementation projects that center the engagement and wellness of people who use drugs including those re-entering communities from incarceration at designated Maryland sites, requiring travel to those sites. They will deliver structured interventions to study and program participants. Mileage reimbursement will be provided at the Federal mileage reimbursement rates.
The Clinical Nurse will have primary responsibility for making long-acting injectable medications (LAIs) used to treat OUD and prevent HIV readily accessible to patients in designated telemedicine sites.
Specific Duties & Responsibilities
- Provide comprehensive nurse case management services to patients engaged in the program in person or via telephone.
- Administer initial comprehensive nursing intake for new patients, document in EMR, coordinate labs, and schedule provider telemedicine appointments in a timely manner.
- Collaborate closely with RNs, NPs, MDs, and Community Health Outreach Workers on the team.
- Primary responsibility for receiving LAI medications, validating inventory logs and ensuring proper storage, and communicating with partner pharmacies.
- Primary responsibility for tracking prescription due dates, scheduled required laboratory tests, coordinating administration scheduling with site personnel, clinicians, patients, and administering LAIs ordered by clinicians across sites.
- Proper documentation of LAI administration in the EMR.
- Provide wound care and perform venipuncture.
- Perform regular follow-up visits with patients in-person or via telephone for: medication education, adherence, radiology and lab appointments, referrals, and Social Determinants of Health.
- Counsel patients regarding harm reduction/disease prevention, health promotion, and treatment plans.
- Deliver structured interventions to study and program participants.
- Direct coordination between Johns Hopkins Specialty Pharmacy, patient insurance, and prior authorization activities is needed to achieve medication approval and delivery.
- Monitor results of lab, radiology, and diagnostic orders before, during, and after the treatment period to ensure that treatment protocols are followed and continuity of care is provided.
- Advocate for patients and coordinate care with insurance companies and outpatient pharmacies to ensure patient needs are being met.
- Promote the program to prospective patients, health care providers, addiction treatment programs, and community organizations via letters, newspaper articles, pamphlets, and in-person visits.
- Maintain regular communication with referring providers and agencies to ensure tight care coordination to optimize patient and referring provider/agency experience.
- Provide program onboarding protocols, training, and logistics support to community partners and function as a resource for program staff to ensure smooth delivery of telehealth services and comprehensive patient care coordination.
- Attend meetings in person at Johns Hopkins in Baltimore as required for educational, programmatic purposes.
- Provide clinical care up to the scope of a registered nurse in the state of Maryland.
- Other duties as assigned.

Minimum Qualifications
- Individual must be a registered nurse, licensed in the State of Maryland.
- Bachelor's Degree in Nursing or related discipline required.
- Minimum of two years of related nursing experience, with additional experience preferred.
- Must have reliable transportation.
- Additional education may substitute for required experience and additional related experience may substitute for required education beyond HS Diploma/Graduation Equivalent, to the extent permitted by the JHU equivalency formula.
Preferred Qualifications
- RN will work in-person and by phone with patients in Allegany, Frederick, and Washington Counties
- Proficiency in Epic electronic medical record system
- Basic data management skills
- Knowledge and interest in HIV/HCV/Substance Use Disorder management and treatment
- Knowledge and interest in public health aspects of infectious disease control
