Community Health Nurse (Interventionist)
We are seeking a Community Health Nurse (Interventionist). The Center for Latino Adolescent and Family Health’s (CLAFH’s) mission is to eliminate health inequities and promote overall wellbeing among Latino adolescents and their families. While our work is Latino-focused, our research and resources are applicable to initiatives aimed at ending inequities for other groups. CLAFH’s core work focuses on developing, evaluating, and disseminating family-based interventions and nurse-driven models of health care delivery designed to mitigate harmful social determinants of health, eliminate health inequities, increase Latino engagement in health care, and promote life opportunities. Specific Duties and Responsibilities
- Triage and deliver substance use/mental health screenings and clinical assessments to Latino and Black youth and their families, and provide/oversee warm hand-offs to services as indicated.
- Ensure care retention through individualized case management, needs assessment, counseling (via text, phone, or in-person), and family support in navigating the behavioral healthcare system.
- Provide overall coordination for a given family’s service package.
- Implement brief and extended interventions for substance use as indicated, appropriate for developmental age and aligned with level of risk.
- Employ brief client-centered, non-judgmental motivational interview techniques that raise the participant’s awareness of their substance use and that promote behavioral change and reduced substance use.
- Work alongside community health workers and families in developing a tailored substance use prevention and treatment plan for the entire household
Minimum Qualifications
- Graduation from an accredited school of nursing with licensure as an LPN.
- Current CPR certification required. Must maintain current licensure and certification during duration of employment.
- Completion of the JHU credentialing process to include verification of CPR certification is required prior to start date.
Preferred Qualifications
- Bronx residency is preferred.
- Bilingual and bi-cultural preferred.
- Flexible schedule (including weekends) is preferred.
- Cultural competence working with Latinos and African Americans.
- Strong commitment to working in communities with health inequities is preferred.
- Candidate must be able to work in a fast-paced, deadline-focused environment, be motivated, be organized, be detail-oriented, be willing to work under the supervision of the project directors and possess the necessary skills to complete tasks efficiently and in a timely manner.
